Just follow the book(but I doubt holding the handle would stop it

Yup, all 20vt's.  Standard feature.  My car lacks heated nozzles, but 
I suspect that's because the car may have been in an accident and the 
hood was replaced with a 5000 hood(I have reason to suspect an 
accident; the missing jets, the wiring mess around the tranny...

Very possible :-)

There's a controller that won't come on below 40-ish, and a 
microswitch that senses the handle being lifted while the door is 
locked.  I believe the Bently has a troubleshooting procedure, but 
I'd try taking off the door panel, hooking up a meter, and lifting 
the door handle.  Check continuity of the heater circuit, then check 
the microswitch with someone inside the car measuring while you lock 
the car and lift the handle.

In any case, mine show "action" after a good 15-20 seconds.  The 
giveaway is a radius of melting snow around the door lock.  It's a 
great source of amusement the first snow of every year(think the 
jetta window-roll-down commercial.)
