I believe I've seen this one on my car before. Came up what seemed to be
errantly. Sounds like your checking faults with the car running; in my case
finally figured out that it was occuring because I was checking fault codes
with the car not running. Doh!

Distributor out of alignment? Did you just do a new distributor, or a
timing
belt? Try rotating the distributor slightly.
Could also be a dirty/damaged reference sensor.
