It pulls straight up.  Grab it with a long nose pliers.

More likely a tired pump, or lost air in the high pressure hose damping
system.  Don't fix it if it ain't broke, or until it is.

You've mentioned about the only one, and that may not have a zerk fitting on
it, and you may not be able to access it without moving the exhaust system
and dropping the shaft.  Any grease is better that no grease.
