Starting Procedures

Gentlemen,
The 3b manual clearly states "Do not depress accelerator pedal while 
starting the 217 hp engine. This applies at any outside temperature."
Roughly 20% of the time my car will not start following this procedure. 
Starts fine if accelerator is depressed. New ISV  (> $300 
w/labor-ouch!) has made no difference. Archive search led to 
unconfirmed recommendations for Ventil Sauber.
As we say in (or under) the hood, what up?
