[A4] Transmission / Engine troubles ?

I was wondering if anyone might be able to help me out or may have experienced these symptoms before.
 
Driving at a constant speed the car will sometimes downshift,  somteimes rev up (all flat roads) with no downshift,  will switch from Tip mode to Auto and will not switch back untill i stop, the tach will bounce around all over the place - all while maintaining a relatively constant speed.  
 
I took it in to an Audi dealer and they could not find anything wrong.  The did say the throttle body was dirty (full of carbon) so they cleaned it.  It ran well for a while and now (1200 kms) has shown these symptoms again (only once or twice).
 
any ideas
 
I have a 1998 A4 1.8T.
