If both of your sensors are going, I would suggest doing a throttle body adaptation. Also, in the case of the positive charge deviation, that usually means a boost leak. For grins, pull the throttle body boot and inspect for damage, as well as your f hose. Both were susceptible to tearing. 

Like everyone else, I recommend pulling the throttle body and cleaning it. In fact, make sure you clean both sides. 

After doing that, do another throttle body adaptation.
