I don't know about getting the threading started, but once you've got the 
piston to the threads, you can turn it with a pair of waterpump pliers on 
the very outside edge of the piston.  You need to be careful to avoid the 
surface covered by the dustboot or your dustboots won't last long.  Perhaps 
a lubricant like vasoline would help get the piston sliding into the seal.

I also have been unable to get the parking brake arm out of the caliper 
from the outside.  I suspect there's something on the inner end of the arm 
that activates the brake and also serves to keep the arm from sliding all 
the way out of the housing.
