After verifying that the ISV works as it should, and then that the cold
start injector triggers fine with proper spray pattern, try:

1) Check/Replace injectors/seals -  poor fuel spray pattern will make cold
starting/running worse.

2) I had good success cleaning the electric fuel pressure regulator maounted
to the fuel distributor.  Clean throuroughly and replace the O-rings.

3) Fuel injector cleaner in gas/BG44K may help.
