90q tranny noises.. suggestions?

Well, I've tried most of the obvious stuff trying to get rid of a terribly
annoying grinding noise that primarily happens when shifting into third and
forth gear. I've noticed that if I match RPM's of tranny and engine really
well I can shift into those gears without grinding..
I've bled the clutch and checked the tranny fluid level (it was down about a
quart!) but it didn't stop the grinding after topping it off. The clutch
pedal feels really good. No spongy feel or anything like that. The engage
and disengage feels smooth. The car has almost 240k on the clock so I'm
guessing the clutch has probably already been replaced but you never know.
One indication I got when bleeding the slave cylinder that at least the
slave had been replaced was the bleed nipple screw was SAE! Suggesting
possibly a rebuilt unit??
In any case, anybody out there have suggestions as to what this could be
(other than a bad transmission and what I mentioned above)? I'm thinking
syncros but wouldn't that be most likely in just one gear?
