Organizing List Topics

Last week there was more discussion regarding repetitive postings / replies
and many ideas toward improving the "list."  I have drafted a "fill in the
blank" format which could be used for most topics along with a completed
sample form with photos and Internet links.  Does anyone have a website who
would be willing to post this so those interested could view / comment /
critique it?
