High Idle, '88 5KSQ

I'm at my wit's end with my high idle problem on my '88 5KSQ.  The idle is
high only when warm.  When cold, it works fine.

1. Idle switch was bad and replaced - no help.
2. ISV replaced - no help.
3. Thermoswitch replaced - no help.
4. Codes read "4444" and "0000".
5. O2 sensor flucuates around 0.05 when cold, 0.02 after a few minutes.
