Urgent help needed please - Audi pro's where are you?

This is definitely not the first thing I would check
because it is SO rare, but I have seen both broken and
loose impellers.  The broken 1 I could kind of
understand.  It is cast iron, but the one that fell
off was wierd.  They are normally a tight interference
fit.  BTW, neither of these were VAG products.
