If the lifters are good, and it goes away when it warms up, it's a cracked
manifold, or blown out exhaust gasket(s)
When the motor (and manifold) warms up, the manifold expands and the crack
closes up. If the noise gets louder with more throttle input, it's
definitely the manifold, or at least there's nothing else it could be :)
