I have no knowledge regarding the fuel, engine output or the study, but 
it's possible the human respiratory system COULD screen out larger 
particulate pollution and not screen out a finer product.  Look at what 
happens when buildings are made airtight for HVAC efficiency and end up 
needing air exchange systems because nobody can spend any time in them 
without becoming ill.
