Your original symptoms indicate that something is preventing this, in
that your car runs either on the small amount of fuel delivered by the
cold start valve (injector), or on a residual pressure leftover from the
"on" position prior to cranking, or cranking.]

I had this problem specifically on the 4kq rally car I sorted out, now runs
beautifully.

Clean the fuel pressure regulator on front of the distributor thouroughly.
 That will cause blockage of fuel to the injectors, or too much fuel if not
clean.
