Which diagram?  The first on the right?

I know little about engines, but I don't see the issue here.
The crank, like a bicycle's pedals make their "power" from
just after 12 o'clock to before 6 o'clock. After detonation.
Leverage may be greatest at 3 o'clock.

It seems that their intake or exhaust ports are closed
and opened by piston travel although their diagram should
show an external valve or there'd be no compression.

VGT says: A reciprocating piston engine generates maximum combustion
chamber pressure when the angle of the crankshaft is least effective.
