My old noisy fresh air fan was much worse in the cold, chirping and 
whining away in the winter, almost unnoticable in the summer.  Put a new 
one in ($100 - force five) this summer when I swapped the interior.  Now 
the only noise I get is the sound of air moving on the higher settings.

It always blew air, though, even when it was noisy.  Except the #2 
setting only worked if I went to #3 first, I suspect the switch.

Your resistor pack may also be at fault, though that usually results in 
the loss of the lower 3 speeds (#4 bypasses the resistor).  The resistor 
  is cheap, since it is an old VW item, and fairly easy to replace 
(behind glovebox).

To replace the fan you have to pull the dash back a few inches on the 
pass. side, not too too tough.  With your knees in the snow...
