200 Q Door Armrest Fasteners

Replaced the regulator in my driver's door, and am now trying to replace the
armrest after putting all the rest of the door panel back together.

But the rearmost allen bolt on the armrest won't tighten. Just keeps
spinning.

I can see *most* of how the fastener is supposed to work....the little
V-shaped metal piece is supposed to secure against the corresponding notch
in the metal tabs protruding from the door panel.

But how does the fastener actually tighten? (If I look at the others, I'm
led to believe that they actually have this thing threading into a wad of
*glue*.)

Also, it appears there is a black plastic sleeve with shoulders supporting
the traveling V-shaped metal piece. Mine has a big notch down the side;
perhaps a prior mechanic's overzealous tightening?

Any thoughts on how to fix this fastener so I can have a tight door handle
again are much appreciated. Thanks,
