The head will not twist on you unless you have
overheated the car, but if you relace the HG, always
have the head milled just to be safe.  Most likely the
gasket is just old or like you say, the bolts are
loose somehow.  Usually this only happens once you
start shoving ~20+ psi of boost though them.
