Is there any pattern?
I've experienced this about 3 of times (in the last 10
years!) and each occasion was just after I'd topped up
the ATF and it was cold. Not investigated any further
as it's been fine for ages.

My experience with the auto tranny in the 1990 200 is that there's a valve
body in the tranny that can stick. When this happens (always while cold) it
won't upshift out of first till it warms up, usually in a mile or two. It
does not indicate a major tranny problem, just a sticky valve body.

In my car, when this happened it would accelerate in first, but not
decelerate - on deceleration it free-wheeled.  However, if I shifted into
first manually it held in gear on both accel and decel.
