Could it be the O2 Sensor?

  While your poor running may have had something to do with a bad O2 sensor, 
the control range of that system is a lot less than the approximate minimum 
125% increase in fueling that you're seeing.   Fuel pressure gone crazy 
maybe?  Injectors blocked open?
