I second the check valve idea. I had this on my '87 5KTQ. As a
workaround, I had a button wired to my fuel pump relay, so I could
initiate the ECU output tests. I found that if it wouldn't start, I
could fire the cold start injector with my button and it would start
after that. It was because the check valve didn't hold the residual
pressure in the system and there wasn't enough fuel on start. I ended
up replacing the fuel pump because it was noisy. The problem went away
(because the check valve on the pump was also replaced).
