Commonly it's failure of the seal on the piston inside the MC.  This can
result from corrosion inside the MC tearing the seal.  A common result of
using the foot to pressurize the system while bleeding it and allowing the
pedal to go beyond normal range of motion because a bleeder is open.
Another good reason for using a pressurized bleeder.
