Whats the history behind this problem?
Did this just happen out of the blue?
Did you just change the coolant, or do some other work on the car?
The problem can be the headgasket. The head gasket will leak gas into the
coolant when the engine warms up, and then you have the "blowout" syndrome.
No, the coolant will not leak into the cylinder to provide you with that
white fog out the exhaust.
