Pretty easy fix, BTDT more than once.  Cut out the offending rubber
section, use it as a guide and get a piece of rigid line (old fuel line,
steel brake like etc.) that fits inside the rubber hose.  Put a couple of
good hose clamps on the rubber line, slide the solid line in the rubber
hose (some silicone helps if it's a little tight), then tighten up the
clamps over the line then you should be all set.

If you want to change the hose, you're in for an expensive and
frustrating experience, just look up over the rear axle at all that rust
and you'll see what I mean.
