If you search the archives, the first hit is exactly what you're  
looking for (I used "fan bearings".)

Make sure you order a set of new mounts, and have replacement nuts/ 
bolts on hand, as well as an assortment of extensions/knuckles; some  
of the bolts are rather difficult to get to.
