Steering wheel shimmy at highway speeds

Lug bolts (2) loose would first sound like a wheel bearing and will lead to 
vibration at all speeds.  BTDT on the 200

We (brother and I) each had the 4000Q and 4000S flutter and vibrate at 65 
when the A frame bushings were worn out.  The shimmy / vibration would stop under 
acceleration or braking.  The worse the bushings got the sooner (lower speed) 
the vibration / shimmy got.  instant fix when bushings only were done.

all these assume your wheels to be balanced like you say.
