Check to make sure your idle switch is closing consistently when the 
throttle closes (unplug the two throttle switches and test with meter). 
  If it is sticking a bit, the ECU won't know to use the ISV to catch 
and control the idle.

That might be a separate problem, but see if you can get the idle 
working right and then tackle the warm start problem.  Does the warm 
start problem occur no matter what you do with the gas pedal?
