Coolant hoses - ever had one fail?

Yes twice actually, first one on my 90 20vq on the motorway middle of winter, it failed where the hose clamp goes on, tiny cut it was spewed coolant all over the engine bay, didn't damage the engine though as i shut it down real qucik. The second was on my Urs4 failed in the same place for the same reason, i replaced it before it went pop this time.

It is rare though, i have not seen it on any other car in 22 years of motoring.
