What brand belt are you using?  It is typical for "American" belts to 
get chewed up and die rapidly, because the "V" is the wrong shape. 
"Euro," metric, belts are what is needed - Continental is one brand that 
works properly.

Have you put a straightedge on it to verify?  I suppose it's possible 
that the mounting bracketry might have been bent.  I don't think there 
is any shimming intended or required to align these things, although 
there might be.  Someone might chime in here...
