I just called my local friendly dealer parts guy... the 613 A number is 
good, described as a "shift selector" (I don't have ETKA installed) and 
lists at 189.55 ("my cost" is 145).  The 615 B number is not a good number.

Rough pricing, yup.  Like I said, several years ago when I bought one it 
was under $100.  Reminds me of the pre and post '84 cgt linkage, the 
early one is several bit of metal and $20 worth of rubber and plastic, 
the later version is one assembly and costs a lot more to replace.

It really is worth doing, but hard on the wallet.

While you're in there, replace the body gasket the rod goes through, and 
if you want you can play at making the shifter throw shorter...
