A hot rod builder will have a custom glass company/guy in their rolodex. 
Down side is that the custom glass benders may not be in your area, and
they'll need the original windshield to bend a new one if they don't have
a pattern.
Don't expect too many bargains though, last custom windshield I had made
for an overseas customer cost around $450.  It was a few years ago, but I
remember the company was in Kentucky somewhere.
