Basically when driving the car and/or left idling.
If I keep the engine revved to about 1500 RPMs, the AC clutch remains engaged and vents blow cold air; I let the revs go to idle and in a few minutes (3-5 tops) the clutch dis-engages and never engages on its own unless I tap it a few times. From the get-go, I have to tap in the AC clutch in order for it to engage.

Those can both be measured - monitor the voltage to the clutch (as close to clutch as possible) during operation/"failure", and check the ground continuity with a load (like a turn signal bulb), not just a meter.
Under what conditions (besides "cold enough") does the system turn off the compressor on your car? Check to make sure everything in that path is working properly.
You may simply need to fix/replace that clutch, of course. Or the whole compressor assembly...
Do you happen to know what that canister-like cylinder on the suction hose is for?
