Door card foam

I have a '91 coupe with padded front door vinyl that is separating from the door card. 

I wish to reglue the vinyl back on. How thick does the foam padding need to be? Any other advice on how I put these vinyls back on the door card?

The door card without the vinyl looks good. However, the interior door handle doesn't fit right without the padded vinyl.
