‘91 coupe quattro idle speed and fuel mileage

Whenever I start my CQ cold, the car stalls unless I keep the idle speed up with the throttle. Warm starts are fine. 

I used to get 360 miles a tank and now it’s down to 300 miles. 

I don’t know if these two things are related, but your advice would be greatly appreciated. 

The cold start valve has been replaced. The ISV has been cleaned and replaced.
