Hammer and chisel.  Unfortunately, this means getting under the car with
enough room to swing.

I've used this method many times.  A little penetrating oil helps things
along too.

You might also try this. I never have, but seen it posted a long time ago.
Try to cut out the crush washer with a hack saw.
