This is a link to the recall of the ignition switches on
some 94-96 Audis, including the S4.

Both of my 95 A6's had the recall done, and both of them promptly
had failed ignition switches again after about 4 more years.

When the switch failed again on the first one, I asked the dealer
about getting it replaced again under the recall, and as you can
imagine the answer was that "no, it has already been done."

I think in 1996 when the recall was done Audi probably rushed to
replace all those defective switches with exactly the same defective
part, and in another few years they all failed again.

I have replaced both of mine in the last 6 months.

recount the whore story in gory detail (sorry, no pictures.)

The switch itself is cheap, about $30, and it took me about 2 hours
to change it.  You have to work in very cramped quarters, and my
hands are not very small, but I managed.
