Dealer sells them, @ $60 if memory serves me.  I believe some local glass
shops can also cut a new mirror for less,.... as long as it isn't the odd
ball dual zone type?

OTOH, if I recall correctly, didn't some lister(s) source a pretty nice dual
pattern driver side with built in wide angle blind spot OEM replacement?
