I lost a wheel at Gingermann Raceway last year, freshly swapped wheels dry torqued to spec, so stuff happens (understatement given the threads of late).
The hubs should be chased with high quality 3 stage (that's 3 separate tools) tap set, M14x1.50. Also remember audi makes different length bolts for different wheels...
Make sure there is no lateral play from the bolts backing out. The best way to test this is to put a bolt in without the wheel, and ck the play. Compare it to one of the other known good hubs. If it's worse, then replacement of the hub is necessary. If you have buggered threads at the face, chances are good the hub is toast.
