You may want to have a good mechanic give you a leak down test on #1 and #2
to see if there has been any damage done by the loss of electrode. You will
also want to listen carefully to the turbo, or if really suspicious pull the
hot side scroll off and carefully inspect the turbine blades for damage.
The little bits and pieces of the electrode went someplace. Plug failure
can be attributed to several causes. First, detonation; second damage (i.e.
dropped etc.) to plug before installation. Last, I guess simply a defective
F5DPOR. Not sure about lean running as a culprit, but if that were in fact
the case, you should also be concerned about other components, like the
pistons &/or valves.
The only things I can think of off hand that might help confirm your
suspicions would be to either buy, or find some one with something like an
Innovate wideband O2 sensor that could check, and log the car's A/F under
operating conditions. e.g., Wide Band O2

Unfortunately, your son isn't the first to have experience F5DPOR plug
failures ~ I've had two. One I'm pretty confident was the product of a bad
tank of gas and consequent detonation. I think it was responsible for a
sizeable "ding" in the turbine blade on that car, not visible from the
exhaust port (one would have had to pull the hotside scroll off to have seen
the chip taken out of the scroll), that eventually lead to the turbos
failure. The other plug failure was/is a mystery.
