Thanks for those who have responded (so far).  I will include your
suggestions.  My brain was working through the night.  I came up with
some of the same ones. (But nowhere near all of them).  I also thought
about these:

Turbo hoses have been replaced/upgraded? (Yes/no)
New turbo hoses are OEM, Samco or ________
Failures: which of the following have failed and if so at what mileage
Cam position sensor
ECU
Power Stage Output (under black fire wall cover)
Coil Pack(s)
Coil to Plug connectors
Spark plugs blowing out of head
Cam timing belt
Alternate: Cam timing belt replacement frequency ____________ mi/km
Water pump
Cam belt tensioner
Fuel pump (in fuel tank)
Exterior door rubber trim

I think I created a MONSTER!!!  Hopefully, it will be worth it.

If you have any more suggestions, please reply to both me and the list
as I will be away from this computer but can pick them up from another
one.
