When I did my timing belt last year I took a look at the tensioner.  I never did replace the bearing but I looked into what would be required.  

The bearing is a standard item available for $5-$10.  I don't remember the exact number.  Something like 6203 but don't quote me on it.  I'd have to look it up at home.  As I recall it's a 40mm OD x 17mm ID x 12mm thick.  If you pop the center cover off the pully you'll see the number on the bearing.  In order to replace it you'll have to drill out the rivets that hold the two halves of the pulley around the bearing and then reinstall with either bolts or rivets (though they are large rivets).  If you go the bolts route make sure you use red Loctite as you don't want those coming loose.
