You may want to try Ozium.  It is an air sanitizer, not a deodorant.
Therefore it kills the odors rather than covering them up.  I have used this
to good effect previously.

You may have to remove the headliner, flooring (carpet or rubber), seat
fabric and padding to fully get the smell out.

So anyone actually ever get rid of it, seems like whatever I do it will

I'm an ex-smoker (3 months without) who smoked in the car, albeit with the
window ALWAYS cracked and NEVER using the ashtray.  I too hate/hated the
smell when not smoking.  Anyway, two things that cut/masked the smell really
well are an apple cut in half, or quarters, and set in the car overnight.
I'm also a fan of incense and periodically burn off a stick or two in the
car.  I do this overnight and keep the windows closed.  One or both might do
the trick for you.

I just purchased a truck, cloth interior and put a contingency on the
purchase that if they couldn't get the smoke smell out that it goes
back.   So far no luck, and I am wondering if it is even possible.
Anyone ever achieved cleansing a car after a smoker has owned it, and worse
smoked in it.  A special chant or maybe a few crystals hanging from the
mirror would do the trick.  I was thinking of trying to humiliate the smoke
out of the car, like "you are weak, you make me sick" actually it does make
me sick.  Good way to ruin an otherwise pristine truck, not a scratch on the
thing, one owner, blab la bla.....
