Randy;

For best results, you have to align the door skin before aligning the window
carrier. The door skin is aligned by adjusting the hinges and door striker.
Unfortunately, the hinges only allow fore and aft adjustment (the gaps at
the front and rear of the door). The latch striker allows the rear of the
door to be adjusted in and out. If the door is twisted as you describe a
hinge may be worn. The bottom hinge on the driver's door is prone to wear;
to test, open the door about one foot and lift the rear edge of the door. If
you can feel play in the hinge it is worn. Another indication of hinge wear
is the door dropping a bit as it comes off the latch striker. The hinges
should be adjusted such that the latch is centred on the striker and doesn't
rub on it until the door is completely closed.

I rebuilt the bottom hinge on my S4 to resolve door fit issues; the pin had
worn elongated holes in the hinge ears resulting in a lot of play in the
bottom hinge.

I was just puzzling over how to get this job done yesterday on my front
passenger door for my 95.5 Avant. But in this case the actual door skin
contour does not quite line up correctly with the read door leading edge
(the front seems to be ok though), AND the window carrier isn't quite
right; the top horizontal line of the door lines up ok, but the the top
half of the door skin protrudes out, while the bottom rear door is
somewhat in relative to the rear door leading edge - its as if the door
is slightly rotated on a longitudinal axis.  I read the procedures Chris
mentioned below for adjusting the window carrier as that task also needs
to be done, but Bentley says that first the door skin needs to match the
contour correctly.

Chris, are you suggesting that the shim will also influence the door
skin tilt, and not just the window carrier?  I'm just a bit confused
with the Bentley info and hoping for a BTDT bit of advice on the door
skin adjustment...
