Hello Lee,
A number of years ago I had a similar mishap on RT46W in Clifton, NJ. I believe I contacted the State DOT. They provided the contractor info. Claim forms were provided & returned with estimate. No photos No adjusters. Very simple transaction from what I remember. Check was in the mail. Payment was made for strut, rim, tire, alignment & labor. All private contractors who win bids with the State, I'm sure are fully insured. Hope this helps. Good Luck, Dan Koropchak

Fargin idyots, the pothole was caused by their lane by lane
application of pavement. This particular pothole did not look new, either.
Any BTDTs appreciated...
