With a long extension on a dremel grinding bit (custom modification in my
case), it can be done without disassembling the internals of the headlight.
Just make sure you apply very little pressure when grinding so you don't
pull the shield off or bend it, and blow metal shaves with clean compressed
air (DO NOT wipe them out, DO NOT use contaminated compressed air (bottled
is the safest)) since the reflective layer in DOT headlights is EXTREMELY
fragile (mechanically AND chemically).
Mine went well on the passanger side until I made the reflective layer rust
within _seconds_ using regular, mostly alcohol-based, glass cleaner to wipe
it. The second trial on the driver side (Yes, I know) was succesful using
clean, bottled, compressed air.
Last piece of advice, it you custom build an extension for a dremel bit,
build it strong (you don't want it to break inside the headlight and carve
some random pattern in it; a very liberal dose of epoxy on precisely aligned
and well cleaned metal rod was enough in my case) and use it as reasonnably
low speed to reduce the stress.
As usual, YMMV, use my advices at your own risk, take into consideration I'm
utterly crazy when it comes to experimentation and, in case something goes
wrong for you, I'm legally inexistant anyway (I think I'm legally safe with
these ;-)

Igor did it to his car when he did the HID conversion.
It involved disassembling the projector to the point where you could access
the "shield" that forms the beam pattern and cutting it to alter the
pattern.
