Don't know if this is on an S6 or an S4, but at least for the S6 make
sure it is snapped in all the way (did you hear the tab click?) and
that the 1/4 turn screw is turned back to the lock position.
Otherwise it will come back out just like butter too.

A lot of folks have ended up replacing theirs twice as a result of not
fully engaging the tab.
