I would imagine the dealer, unless there's a separate kit for that, but I've 
never heard of one. The TRW have a reputation for lasting, while the Meyle 
don't.

I have heard that replacing the various nut, bolds and sleeves when 
replacing the bushings is critical.  If I go with TRW it does not appear 
those parts are included.  Where would I find them?

Sounds like it is probably worth replacing the entire control arms.  I have 
been looking at the TRW and Meyle HD parts.  The TRW have a better 
reputation and better price, but the Meyle come with all of the hardware 
required.

If the front control arm is all you intend to replace, then no special tools 
are needed.  The sway bar nut may be larger than most common wrench sets 
provide and the hold-down straps mentioned in the FAQ are helpful in 
aligning the sway bar back in place on the new control arm, but no real 
special tools are needed.  I have a 6' iron bar I use to lever the control 
arm into position to connect the ball joint when working by myself.

The bushings are usually the first thing to go, so you could replace the 
inner bushing and control arm bushings if the ball joint is in good 
condition.  This is usually a matter of cost and/or effort involved in the 
job.  The TRW aftermarket control arms have received good reviews from most 
listers as more cost effective than the dealer supplied parts, but the work 
involved in the replacement or labor costs at a shop usually have most 
owners wanting to make sure the job lasts, so most will replace all the 
suspension wear items at the same time.

Best practices dictate that you replace the corresponding suspension parts 
on both sides.
