All the bearing sets that I've used over the last 10years have had the 
integral thrust bearings.

I replace them if the engine has done more than 150K miles. Remove the back 
plate - if it's scored then replace the pump. You _can_ replace the pump 
with the engine in the car but it's not easy - I'd change it now.
