[V8] axle bolt

I bought a wheel bearing kit for my 90 V8.  The axle bolt that came with it doesn't look like what's in there now. The one in there now has a 27mm cap head on it and a thick flat washer.  What came with the kit is a hex key head with no washer, and the shaft of the bolt spreads into a cone shape where it attaches to the head.  I have 2 concerns, the different shape of the mating surface, and trusting the hex head with 148 ft lbs plus one quarter turn. The manual says always replace. Any BTDT?
